The forms and guides below are for the academic year 2010/11.

Forms and guides for academic year 09/10 are available from academic year 2009/10.

You should send your completed forms to your local authority. To identify your LA and their contact details please refer to the LA finder.

2010/11 Full Time Students

These guides provide detailed information on all aspects of the financial support available during 2010/11:

Loan request forms

Students who have already applied for support and wish to amend the loan amount they requested should complete either:

Birth Certificates or Adoption Certificates for evidence

We recommend that you send in your original passport for evidence.

You can also send in your Birth or Adoption Certificate for evidence. You must also complete and return the form below.

This form contains two separate declarations. The Applicant's Declaration of Identity must be signed by the student to confirm their identity. The Identity Confirmation Form is to verify your identity and must be completed by a person of good standing in the community who has known you for at least two years. Please note: this person must be currently in employment, and they must supply a phone number for their place of work.

Disabled Students Allowance

If you have completed or are completing your application for student finance and you also want to apply for Disabled Students' Allowances, you should complete the slimline version of:

Full-Time Students who wish to apply for help with childcare costs and other support, will require these forms:

Part-time Students 2010/2011

Part Time students wishing to apply for support should download:

Complete the PTG1 form and take it with you to your university or college when you start your course.

The administration office will complete the rest of the form (for new students - waiting until you’ve attended for two weeks), confirming:

  • that you’re a student there;
  • how intensive your course is; and
  • what the tuition fees are.

You will then need to send the form to your Local Authority.
